| 摘 要： | 目的探讨胃癌患者外周血、门静脉血及癌组织标本p53基因突变或异常表达的临床意义。方法应用PCR-SSCP法检测73例手术治疗的胃癌患者外周血和门静脉血中p53基因的突变,并用免疫组织化学法检测胃癌组织中p53蛋白的异常表达。结果73例胃癌患者中有7例检测到外周静脉血p53基因突变,有29例检测到门静脉血p53基因突变,有51例检测到胃癌组织中p53蛋白的异常表达;胃癌组织中的p53蛋白阳性表达率（69.86%）和门静脉血中p53基因突变检出率（39.73%）均显著高于外周血的检出率（9.59%）。经中位24个月随访,其中有16例出现术后异时性肝转移,这些患者外周静脉血、门静脉血以及胃癌组织中的p53基因突变检出率或p53蛋白阳性表达率均显著高于无肝转移者,而且7例外周静脉血、门静脉血及胃癌组织中同时检测出p53基因突变或p53蛋白阳性表达的患者均发生肝转移。结论胃癌患者外周血中p53基因突变者术后发生异时性肝转移的可能性显著增加,外周静脉血联合门静脉血以及胃癌组织中p53基因突变检测有助于判定胃癌患者的预后。 |
