青年结直肠癌患者临床病理特征分析

目的分析青年结直肠癌患者的临床病理特征，提高青年结直肠癌的诊治水平。方法收集2004年1月至2005年12月诊治的608例结直肠癌患者的临床资料，按年龄〈35岁为青年组（47例），36～60岁为中年组（264例），〉61岁为老年组（297例），分析不同年龄组患者其发病情况、性别、发病部位、组织学分级、大体类型、首发症状、分期、治疗、预后等临床病理特征的差异。结果与中、老年组比较，青年组患者的临床病理特征为：首发症状典型，如以腹痛、便血、排便习惯改变、肠梗阻等；确诊时病理分期较晚，以Ⅲ、Ⅳ期为主；组织学分化程度以中分化腺癌和低分化腺癌为主；大体病理类型以菜花型、溃疡型为主；姑息手术所占的比例较高；预后较差，3年生存率低于中、老年组。结论年青结直肠癌患者确诊时分期较晚、组织学分化程度差，根治手术率低，预后差，临床应给予早期诊断和积极治疗。

Clinicopathological Characteristics Analysis of Younger Colorectal Cancer Patients

Objective To investigate elinieopathological characteristics of younger eoloreetal cancer patients and we will improve the knowledge of diagnosis and treatment based on it. Methods The clinieal data of 608 eases the differences of Clinieopathologieal eharaeteristies in Guangdong Provincial people＇s hospital from Jan. 2004 to Dee. 2005 were analysed among younger patients,middle age and elderly patients retrospectively. Results Coloreetal cancer patients with different groups showed no difference in location,while eolorectal patients with different groups showed differences in histology, first symptoms, radical resection rate, pathologie stage. Conclusion It is important for physicians to recognize the advanced stage, poor differentiation and poor outcome of CRC in younger population and consider an aggressive approach to diagnosis and early treatment.
