腹腔镜阑尾切除术82例体会

目的：总结腹腔镜阑尾切除术的临床经验，探讨实用、有效、简便的手术方法。方法：回顾总结82例腹腔镜阑尾切除术的临床资料。结果：79例顺利完成腹腔镜阑尾切除术，3例中转开腹，其中1例术中损伤盲肠致穿孔，1例超声刀误伤回肠穿孔，1例因网膜粘连紧密，无法用腔镜器械剥离而中转开腹，手术时间最短16min，最长135min，平均手术时间42min,术后切口感染1例,术后住院时间平均2．6d。结论：腹腔镜阑尾切除术创伤小、痛苦少、恢复快、住院时间短，与开腹阑尾切除比较具有诸多优越性，应成为阑尾炎的首选治疗方法。

Clinical experience of 82 cases with laparoscopic appendectomy

Objective:To summarize clinical experience of laparoscopic appendectomy and investigate an useful,effective and convenient operation procedure Methods:The clinical data of 82 patients with laparoscopic appendectomy were reviewed retrospectively Results:The operation of 79 cases were successfully finished.3 cases were switched to laparotomy,among them,1 case for the perforation of ileum because of the injury by the ultrasonic dissector,1 case for tight adhesion of omentum and unability to be stripped using laparoscopic apparatus.The shortest time of operation was 16 minutes and the longest was 135 minutes,with all average of 42 minutes.The time of stay after operation is 2 6 days on the average Conclusion:Laparoscopic appendectomy has little wound,little pain,rapid recovery,and short stay.Compared with laparotomy,it has great superiority and will become the primary treatment for appendicitis
