血清NSE、CYFRA21-1检测对肺癌诊断的临床应用

目的　探讨血清NSE、CYFRA2 1 1检测对肺癌诊断的临床应用价值。方法　检测 173例肺癌、36例肺良性疾病患者和 5 2例正常对照血清NSE、CYFRA2 1 1的水平 ,NSE采用电化学发光免疫分析法 ,CYFRA2 1 1采用放射免疫分析法。结果　肺癌组病人血清NSE、CYFRA2 1 1水平均明显高于肺良性疾病组和正常对照组 (P <0 0 0 1) ,NSE水平升高以小细胞肺癌 (SCLC)最为显著 (86 72± 98 2 μg L) ,灵敏度为 82 4 % ,特异性为 95 5 % ;而CYFRA2 1 1水平升高则以非小细胞肺癌 (NSCLC)最为显著(18 6 7± 2 1 6 1μg L) ,灵敏度为 6 1 4 % ,特异性为 95 5 %。NSE、CYFRA2 1 1联合检测肺癌的灵敏度为 91 9% ,特异性为 95 2 % ,阳性预测值为 95 2 % ,阴性预测值为 85 1% ,实验有效率为 91 6 %。结论　血清NSE、CYFRA2 1 1检测对肺癌诊断均有重要意义 ,NSE、CYFRA2 1 1分别有助于辅助诊断SCLC、NSCLC ;两项联合检测可提高肺癌诊断的临床价值 ,优于单项检测。

Clinical application of serum NSE and CYFRA21-1 determinations for diagnosis of lung cancer

Objective To investigate the clinical application value of serum NSE and CYFRA21-1 determinations in diagnosis of lung cancer.Methods The serum NSE and CYFRA21-1 levels of 173 patients with lung cancer,36 patients with benign lung disease,and 52 healthy people were determined.Serum NSE was determined by electrical chemiluminescent immunoassay(ECLIA),and serum CYFRA21-1 was determined by radioimmunoassay(RIA).Results The serum NSE and CYFRA21-1 levels of lung cancer were all significantly higher than those of benign lung disease and normal control(P<0.001),especially the NSE level in small cell lung cancer(SCLC) was elevated most markedly,sensitivity and specificity were 82.4% and 95.5%,respectively;and the CYFRA21-1 level in non small cell lung cancer(NSCLC) was elevated most markedly,sensitivity and specificity were 61.4% and 95.5%,respectively.When the combining detection of serum NSE and CYFRA21-1 was used for the diagnosis of lung cancer,sensitivity,specificity,positive predictive value,negative predictive value,and experimental validity were 91.9%,95.2%,95.2%%,85.1% and 91.6%,respectively.Conclusion Serum NSE and CYFRA21-1 are of clinical importance for the diagnosis of lung cancer,in single,especially,NSE and CYFRA21-1 are helpful for the diagnosis of lung cancer SCLC and NSCLC,respectively.Compared with the single item,the combining detection of serum NSE and CYFRA21-1 can improve clinical value for the diagnosis of lung cancer.
