ADA、CEA和Ft联合检测对结核性和癌性胸水的鉴别诊断价值

目的：探讨腺苷脱氨酶（ADA）、癌胚抗原（CEA）和铁蛋白（Ft)对结核性和癌性胸水的诊断价值。方法：检测30例结核性胸膜炎和21例癌性患胸水中的ADA、CEA和Ft含量。结果：单项检测：ADA诊断结核性胸膜炎的敏感性为86.7%，特异性为80.1%，准确度为86.7%；CEA和Ft诊断癌性胸水的敏感性分析为76.2%和66.7%，特异性为93.3%和86.7%，准确度为88．9％和77．8％。联合检测：以三项同时符合结核或癌性胸水为诊断依据时，结核性和癌性胸水的敏感性分别为70％和52．4％，特异性和准确度均为100％。结论：ADA、CEA和Ft联合检测对结核性和癌性胸水有较高的鉴别诊断价值。

The value of ADA, CEA and Ft in the differential diagnosis of malignant and tuberculous pleural effusion

Objective:To investigate the value of ADA,CEA and Ft in the differential diagnosis of malignant and tuberculous pleural effusion Method:The activity of ADA,CEA and Ft was tested in 32 patients with tuberculous pleural effusion and in 21 patients with malignant pleural effusion Result:The sensitivity,specificity and diagnostic accuracy rate of ADA for the tuberculous pleural effusion were 86 7%,80 1% and 86 7% respectively The CEA rates for malignant pleural effusion were 76 2%,93 3%and 88 9% respectively,while the Ft rates were 66 7%,86 7% and 77 8% respectively When the three tests were used in combination,sensitivity rate was 70% for TB and 52.4% for malignant pleural effusion;the specificity and diagnostic accuracy rates of the tow groups were 100% Conclusion:ADA,CEA and Ft are valuable in differentiating between mailgnant and tuberculous pleural effusion
