肝脏瞬时弹性探测仪在诊断慢性乙型肝炎患者不同分期肝纤维化中的应用价值

目的 探讨肝脏瞬时弹性探测仪(FibroScan)在诊断慢性乙型肝炎患者不同分期肝纤维化中的应用价值.方法 选取200例慢性乙型肝炎患者,均行病理活检诊断肝纤维化分期,同时采用FibroSean检测肝弹性值.比较不同分期患者的肝弹性值,分析肝弹性值与肝组织纤维化分级的相关性.采用受试者工作特征曲线法评估FibroScan诊断不同分期肝纤维化的价值.结果 S0、S1、S2、S3、S4期患者的肝弹性值依次升高(P<0.05),肝弹性值与肝纤维化分期呈正相关(P<0.05).FibroScan诊断S1、S2、S3、S4期肝纤维化的曲线下面积分别为0.931、0.955、0.968、0.954,特异度及灵敏度均大于90％.结论 FibroScan检查对慢性乙型肝炎患者不同分期肝纤维化均具有较高的临床诊断价值.

Application value of liver instantaneous elastic detector for diagnosing different stages of liver fibrosis in patients with chronic hepatitis B

Objective To investigate the application value of liver instantaneous elastic detector (FibroScan) for diagnosing different stages of liver fibrosis in patients with chronic hepatitis B (CHB).Methods A tota1 of 200 patients with CHB were enrolled.All patients underwent pathologic biopsy for staging of liver fibrosis .Liver stiffness values were measured by FibroScan in all patients .The liver stiffness values were compared among the patients with different stages .The correlation of liver stiffness value with fibrosis classification of liver tissues was analyzed .The values of FibroScan for diagnosing different stages of liver fibrosis were assessed by receiver operating characteristic curve . Results The liver stiffness values were elevated in the order of the patients with S 0,S1,S2,S3 and S4(P<0.05).Liver stiffness value positively correlated with liver fibrosis stage (P<0.05).The areas under the curve of FibroScan for diagnosing liver fibrosis of S1,S2,S3 and S4 were 0.931,0.955,0.968 and 0.954 respectively,and its specificity and sensitivity were more than 90％.Conclusion FibroScan obtains a higher clinical diagnostic value for diagnosing different stages of liver fibrosis in CHB patients .
