回肠膀胱术与原位回肠新膀胱术近期临床疗效比较

目的：探讨分析全膀胱切除术后两种尿流改道术式的临床效果差异。方法：回顾分析57例膀胱癌患者，其中行回肠膀胱术35例，原位回肠新膀胱术22例，对两组患者在出血量、手术时间、术后平均住院天数、住院费用以及术后早期并发症发生率等方面的情况进行统计学比较分析。结果：回肠膀胱术组的手术出血量l000（650，1400）ml较原位回肠薪膀胱术组500（300，6001ml为多，而在术后平均住院天数和住院费用方面，回肠膀胱术组则少于原位回肠新膀胱术组．分别为25d和39d，21078．74（18840．13，28140．02）元和36483．13（33496．01，45464．16）元，差别均具有统计学意义（P〈O．05）。两组在手术时间（393±74与416±113）min】、术后早期并发症发生率（62．9％与63．6％）、围手术期死亡率（2．9％与4．6％）等方面无统计学差异（P〉O．05）。结论：回肠膀胱术与原位回肠新膀胱术都是目前较好的膀胱全切术后尿流改道方式，二者各有优缺点，应根据患者的个体情况选择合适的手术方式。
