类风湿关节炎患者血清人软骨糖蛋白-39的检测和意义

目的：探讨血清人软骨糖蛋白-39（HC gp-39）在类风湿关节炎（RA）患者中的诊断意义。方法：对40例RA患者，15例系统性红斑狼疮（SEE）患者及20例健康人，应用夹心酶联免疫吸附法（sandwich—ELISA）检测血清HCgP-39水平。同时测定RA患者类风湿因子（RF）、红细胞沉降率（ESR）和C反应蛋白（CRP）的浓度。并记录RA患者的压痛关节数、肿胀关节数、晨僵时间、双手X线、疾病活动指数28（DAS28）。分析它们与HC gp-39的相关性。结果：RA患者组血清HC gp-39的浓度为01．95±18．54）ng／ml，高于正常对照组（18．93±13．49）ng／ml（P〈0．01）和SLE组（20．75±11．26）ng／ml（P〈0．05）。而后两组之间差异无统计学意义（P〉0．05）。25例RA活动期患者血清HC gp-39的水平为（37．92±19．58）ng/ml，15例非活动期患者为（22．00±11．41）ng/ml，组间比较差异有统计学意义（P〈0．01）。HC gp-39水平与RA患者的压痛关节数、肿胀关节数、ESR、CRP、RF、晨僵时间、DAS28呈正相关（P〈0．05），与年龄、性别、病程、X线分期无明显关系（P〉0．05）。结论：RA患者血清中HC gp-39水平异常增高，提示血清HC gp-39可以作为反映RA病情活动性及诊断的指标之一。

Detection and significance of serum Human cartilage glycoprotein-39 in patients with rheumatoid arthritis

Objective： To explore the diagnostic value of serum HC gp-39 in patients with rheumatoid arthritis （RA） o Methods： The serum levels of HC gp-39 were measured with sandwich-ELISA in 40 patients with rheumatoid arthritis （RA）,15 patients with systemic lupus erythematosus （SLE） and 20 normal controls. The serum levels of erythrocyte sedimentation rate （ESR）, rheumatoid factor （RF） and C-reactive protein （CRP） were examined. The number of painfull joints, the number of swollen joints, hands X-ray and disease activity score 28 were recored. The correlation among them and HC gp-39 were compared. Results： Serum HC gp-39 levels of RA patients were （31.95 ＋18.54）ng/ml, higher than that of normal controls （ 18.93＋ 13.49）ng/ml （P〈0.01 ） and patients with SLE （20.75＋ 11.26）ng/ml（P〈0.05 ）.There was no difference inthe concentration of HC gp-39 between the normal contral group and the SLE group （P 〉 0.05）. Serum levels of HC gp-39 were higher in 25 active patients （37.92＋19.58）ng/ml than that in 15 inactive patients （22.00＋11.41 ）ng/ml （P〈0.01）. HC gp-39 levels were positively correlated with the number of the painfull joints,the number of swollen joints, RF ,CRP, ESR, morning stiffness and DAS28 （P 〈 0.05）; but were not correlated with age, gender, disease course, and X-ray stage （P 〉 0.05）. Conclusion： The present date suggests that the levels of HC gp-39 are high in patients with RA. High serum level of HC gp-39 indicates high disease activity. We can use HC gp-39 as a laboratory marker of RA.
