颌面部犬咬伤的治疗及缺损修复

目的:探讨颌面部犬咬伤的临床特点及处理方法。方法:回顾1990年2月～2006年10月间接受手术治疗的犬抓咬伤患者62例,对其年龄分布、损伤类型、创面处理和软组织缺损的修复方法进行分析。结果:本组62例患者中,小于15岁者占72.6%;损伤类型以单纯软组织损伤为主,合并软组织缺损者占33.9%;21例软组织缺损采用邻近皮瓣一期修复。结论:本病儿童多见,加强儿童的自我保护意识和有效的防范措施十分必要。严格的创面处理及免疫制剂的应用是减少该病发作的两大重要环节。对伴有软组织缺损、影响美观的患者,各种邻近带蒂组织瓣一期修复可获得满意的效果。

Treatment and reconstruction of defect of dog bites in maxillofacial region

Objective: To discuss clinical characteristic and treatment of bite wound by dog in maxillofacial region. Methods: By retrospecting 62 patients treated for bite wound by dog between February 1990 and October 2006, the distribution of age,the type of wound,the management of raw surface and the reconstruction of defect of soft tissue were analyzed. Results: Of all the 62 patients, 45(72.6%) patients were aged 15 years and younger. Injury of soft tissue without defect was the main type of wound, while injury of soft tissue with defect which was reconstructed primarily by pedicle flaps account for 33.9% (21 of 62). Conclusion: We should strengthen consciousness of self-protection of children and take some necessary measurements to prevent it. Managing raw surface strictly and using immune drugs can diminish postoperative complications effectively. Primary reconstruction of defect of soft tissue by pedicle flaps can get satisfying outcome.
