摘    要:目的 探讨非小细胞肺癌(NSCLC)组织肺耐药相关蛋白(LRP)表达及其临床意义。方法 应用组织芯片技术结合免疫组化PV-9000二步法检测术前未经化疗的72例NSCLC组织中LRP的表达,并采用图像分析技术进行定量测定。结果 LRP主要在NSCLC的细胞浆表达,细胞膜有少量表达,总阳性表达率为77.78%。LRP表达强度与组织学类型有关,在腺癌中阳性颗粒的平均吸光度与积分吸光度均高于鳞癌(t=6.977、6.114,P〈0.001);LRP表达强度与肿瘤细胞分化程度有关,随分化程度的降低平均吸光度与积分吸光度均降低(F=4.717、5.324,P〈0.05);其表达强度与临床分期、有无淋巴结转移、肿瘤大小及年龄无关(F:1.901、2.165,t=0.837-2.165,P〉0.05)。结论 检测LRP在NSCLC组织中的定量表达,能准确地反映NSCLC组织的耐药性及对化疗的敏感性,在指导肿瘤化疗方面具有重要的临床意义。

Abstract:Objective To study the expression of lung resistance-related protein(LRP) in non-small cell lung cancer(NSCLC) and its clinical significance. Methods NSCLC specimens(n=72) were examined by the combined techniques of tissue chip and immunohistochemistry.The expression of LRP was quantitatively assessed by computer image analysis system.Chemotherapy was not performed in all cases before surgery. Results Expression of LRP was found mainly in the cytoplasm of NSCLC,few in cellular membrane.The total positive rate of LRP expression was 77.78%.The intensity of LRP immuno-staining was related to the tissue type and differentiation of the cancer.The average density and accumulated density of LRP were higher in adenocarcinoma than in squamous cell carcinoma(t=6.977,6.114;P<0.001),and higher in well differentiation group than in poor differentiation group(F=4.717,5.324;P<0.05).They were not related to the clinical stage,lymph node metastasis,patient's age or tumor size(F=1.901,2.165;t=0.837-2.165;P>0.05). Conclusion Quantitative analysis of LRP expression in NSCLC can well reflect the drug resistance of NSCLC and predict the sensitivity of cancer to chemotherapy.It is of clinical significance to guide the chemotherapy.
