支气管哮喘病儿血清IL-17、IgE与EOS计数变化及意义

摘    要:目的探讨支气管哮喘病儿血清白细胞介素17(IL-17)、免疫球蛋白E(IgE)水平及嗜酸性粒细胞(EOS)计数与哮喘发生、发展的关系。方法检测82例轻中度哮喘病儿(哮喘组)血清IL-17、IgE水平以及EOS计数,以20例健康体检儿童作为健康对照组。结果哮喘组血清IL-17、IgE水平及EOs计数均高于健康对照组,差异有显著性(t=4.857-8.379,P〈0.01)。哮喘病儿发作期血清IgE水平明显高于缓解期和健康对照组,缓解期高于健康对照组(F=32.825,q=16.05~35.68,P〈0.01);发作期血清IL-17水平及EOS计数高于健康对照组,差异有显著意义(F=38.781、79.471,q=3.56~8.35,P〈0.05),与缓解期比较差异无显著意义(P〉0.05)。哮喘病儿血清IL-17与EOS计数呈正相关(r=0.846,P〈0.01),IL-17、EOS计数与IgE之间无明显相关,但哮喘急性发作时IL-17与IgE呈正相关(r=0.438,P〈0.05)。结论IL-17可能参与介导哮喘的嗜酸性粒细胞性炎症,联合检测血清IL-17、IgE水平以及EOS计数对哮喘病情判断及治疗有重要临床意义。

Abstract:Objective To study the association of serum levels of IL-17, EOS and IgE with the occurrence and development of bronchial asthma (BA) in children. Methods Serum levels of IL-17 , IgE and EOS were detected in 82 children with mild-or moderate asthma, 20 children who had a health examination were served as healthy controls. Results The serum IL-17, EOS and IgE in the BA group were significantly higher than that in the healthy control group (t=4.857-8.379,P〈0.0l). The levels of IgE in the period of onset were higher than that in the remission group and the control group, that in the remission were higher than that in the healthy control (F = 32. 825, q = 16.05 - 35.68, P 〈 0. 01). In the period of onset, the levels of IL-17 and EOS were higher than that in the healthy controls (F = 38.781,79.4 71;q = 3.56- 8.35; P〈0.05) , as compared with that in the remission, the difference was not significant (P〉0.05). In the sick kids, the serum IL-17 was positively correlated with EOS (r= 0.846,P〈0.0l), and no obvious correlation was noted among IL-17 , EOS and IgE, but, when acute episode, IL-17 was positively correlated with IgE t r = 0.438, P〈0.05). Conclusion IL-17 is probably involved in mediating eosinophilic inflammation in asthma. A combined detection of IL-17, IgE and EOS is of great significance in prediction and therapy of the disease.
