慢性免疫性血小板减少症患者外周血淋巴细胞亚群及相关细胞因子的研究

目的本研究检测慢性免疫性血小板减少症(CITP)患者淋巴细胞亚群数量以及相关细胞因子的变化,初步探讨它们在CITP发病机制中的可能作用。方法选择40例CITP患者,正常健康体检者20例,检测外周血单个核细胞T淋巴细胞,Th细胞、Ts细胞、B淋巴细胞和NK细胞的表达率,测定CD~(4+)细胞中Th17细胞、Th22细胞、T-reg和Th1细胞的表达率;应用酶联免疫吸附法(ELISA)检测血浆中IL-17、IL-22、IL-6、TNF-α的水平。结果与正常对照组相比,T淋巴细胞总数、Th细胞、Ts细胞、B淋巴细胞、NK细胞,Th17细胞,Th22细胞表达率,IL-17、IL-22水平、IL-6、TNFα的表达水平差别均无统计学意义(P0.05),Th1表达率上升,T-reg表达率下降,差别均有统计学意义(P0.05)。结论慢性ITP患者免疫自稳能力下降和Th1功能亢进可能在其发病过程中起主导作用,而Th17,Th22可能与慢性ITP的发生无关。

Investation of Lymphocyte Subsets and Related Cytokines of Peripherm Blood in Patients with Chronic Immue Thrombocytopenia

Objective This study was aimed to detect the level of the lymphocyte subsets in patients with chronic immue thrombocytopenia(CITP) and to analyse the level of related cytokines.Methods A total of 40 cases were taken as the research group and 20 healthy persons were served as the control group.Peripheral blood mononuclear cells (PBMNC) were separated,the percentages of Th1,Th17,Th22 and Treg cells were detected by flow cytometry,and the IL-6,TNF-α,IL-17,and IL-22 levels from serum also were determined by EL1SA.Results Compared with healthy controls,There were no statistically significant difference between the two groups for the percentaged of T lymphocyte 、Ts cell、B lymphocyte 、NK cells,Th17 cells,Th22 cells and the serum level of interleukin(IL)-17,IL-22,IL-6 and TNF-α (P ＞ 0.05).While Thl cell being increased,regulatory T cell ratio being reduced in CITP patients (P ＜ 0.05).Conclusion Hyperfuction of Th1 cells and depression of immunological homeostasis may play an important role in the process of CITP,while Th17,Th22 may be unrelated.
