青霉素特异性IgE检测及其临床意义

利用单克隆抗体及生物素－亲和素放大系统检测青霉素抗原特异性ＩｇＥ，并探讨其临床意义。结果５９例青霉素Ⅰ型变态反应患者中青霉素特异性ＩｇＥ总阳性率为８８．１１％（５２／５９）；１４例门诊皮试假阳性患者特异性ＩｇＥ阴性，使用青霉素无不良反应；９例猩红热样皮疹青霉素过敏患者中１例特异性ＩｇＥ阳性；而３１５例非青霉素过敏者，其ＢＰＯ特异性ＩｇＥ均阴性，且其中部分患者使用青霉素亦无过敏反应。本文方法有特异性强、敏感性较高的特点。

DETECTION OF PENICILLIN SPECIFIC IgE AND ITS CLINICAL SIGNIFICANCE

We detected penicillin(BPO,6-APA,BPE)specific IgE by ELISA with mono-clonal antibodies(McAb)and biotin-avidin system(BAS),and discussed its clinical signifi-cance.The results showed that the total positive rate of penicillin specific IgE in the sera of patients with immediate reaction to penicillin was 88.11%(52/59).Penicillin specific IgE in the sera of 14 patients with false positive skin test were all negative,penicillin spe-cific IgE was false positive in the serum of only one patient with scarlatiniform exanthem after penicillin treatment.The BPO specific IgE in 315 normal subjects were all negative and some of these patients had no side reactions after penicillin treatment.These methods are more specific and sensitive than other methods.
