胎盘早剥的早期诊断

目的:探讨胎盘早剥的临床表现、早期诊断指标,以减少漏诊与误诊。方法:对中日友好医院17年中44例胎盘早剥的病例进行回顾性分析。据胎盘剥离面积分为轻型组和重型组,观察指标包括胎盘早剥的症状、体征、B超诊断率、分娩方式和母婴预后。结果:轻型胎盘早剥临床表现不典型,以腹痛、阴道出血多见。重型胎盘早剥临床表现典型,持续性腹痛、宫缩间期不松弛,阴道出血多见,B超诊断率高,漏诊误诊率低,但预后差。结论:根据临床表现及辅助检查进行动态观察,做到早期诊断、早期干预,避免漏诊误诊,改善预后。

Early diagnosis of placental abruption

Objective:To explore the clinical manifestation and early diagnosis of placental abruption and to reduce the rates of missed diagnosis and misdiagnosis.Methods:Forty four cases of placental abruption in China-Japan Friendship Hospital during recent 17 years were retrospectively analyzed.The cases were divided into mild type and severe type.The observation index included symptoms,signs,ultrasonic diagnosis rate,methods of delivery and the outcome of mothers and infants.Results:The clinical manifestation of mild placental abruption was mostly atypical.Its main clinical manifestation included abdominal pain and vaginal bleeding.The clinical manifestation of severe type was typical.It included continuous abdominal pain,the uterine not relax between contractions and vaginal bleeding.It had higher ultrasonic diagnosis rate.But the outcome of pregnancy was poor.Conclusion:According to the clinical manifestation,assistant examination and continuous observation,early diagnosis and proper treatment can improve prognosis as well as reduce missed diagnosis and misdiagnosis.
