2007年临床分离常见细菌耐药陛监测结果分析

摘    要:目的:分析中日友好医院2007年临床分离常见细菌对各类抗菌药物的耐药性,指导临床更加合理用药。方法:应用VITEK-2进行菌株鉴定和药敏试验,用WHONET5.4软件进行数据分析。结果:2007年我院各种临床标本共分离2276株非重复菌株,其中大肠埃希菌(16%)、铜绿假单胞菌(10%)、金黄色葡萄球菌(7%)、肺炎克雷伯菌(6%)、鲍曼不动杆菌(5.7%)、粪肠球菌(5.7%)分别排前6位。亚胺培南、哌拉西林,三唑巴坦、阿米卡星对肠杆菌科细菌抗菌作用较强.敏感率〉94%。发现1株大肠埃希菌对亚胺培南耐药。大肠埃希菌、肺炎克雷伯菌超广谱β-内酰胺酶的检出率分别为57.6%、26.2%。甲氧西林耐药的金黄色葡萄球菌和甲氧西林耐药凝固酶阴性葡萄球菌的检出率分别为57.3%、61.0%;未发现对万古霉素和替考拉宁耐药的葡萄球菌和肠球菌,但有1株粪肠球菌对万古霉素中度敏感。碳青霉烯类、阿米卡星对铜绿假单胞菌、鲍曼不动杆菌抗菌活性相对较强,敏感率〉80%。结论:由于临床分离菌株对常用药物的耐药率不断增高,应当加强病原菌的耐药性监测。

Bacterial resistance surveillance analysis of most common clinical isolates in 2007

Abstract:Objective:To investigate the antimierobial resistance of clinical isolates collected in China-Japan Friendship Hospital in 2007.Methods:All strains collected were identified and the antimicrobial susceptibility were determined with VITEK-2.The data were analyzed with WHONET5.4 software.Results:From January to December 2007,2276 consecutive and non-repetitive bacteria were collected from the hospital.Most common isolates were Escherichia coli,Pseudomonas aeruginosa,Staphylococcus aureus,Klebsiella pneumoniae,einetobacter baumannii,Enterueocoi faecalis.Escheriehia coli and Klebsiella pneumoniae were highly sensitive to imipenem, piperacillin/tazohactam,amikacin,the sensitive rates being over 94%.One strain of Escherichia coli was resistant to imipenem.The incidence of methicillin resistant Staphylococcus aureus,methicillin resistant coagulase negative Staphylococci,extended spectrum β-lactamases producing strains of Escherichia coli,Klehsiella pneumoniae,was 57.3% ,61.0% ,57.6%,26.2%,respectively.No isolates of staphylococci and enterococei were resis- tant to vancomycin and teicoplanin,but 1 strain of Enterococoi faecalis was mid-sensitive to vancomycin. Pseudomonas aeruginosa and Acinetobacter baumannii were sensitive to imipenem,meropenem,amikacin,the sensitive rates being over 80%.Conclusion:It is important to survey antimicrobial susceptibility of the most common clinical isolates.
