30例双胎输血综合征声像图特征的回顾性分析

目的探讨产前超声检查对双胎输血综合征的预测、诊断及价值。方法对2006年8月至2011年3月在本科确诊,并有最终随访结果的30例双胎输血综合征孕妇的60个胎儿的超声表现进行回顾性分析。结果 30例孕妇中共60个胎儿,17例孕妇的胎儿S/D增高,而且受血儿较供血儿增高明显;14例孕妇的受血儿心脏增大,1例孕妇供血儿心脏增大;8例孕妇的供血儿为贴壁儿;20例孕妇接受胎儿镜治疗,15例较治疗前有明显好转;脐带帆状入口和边缘入口的胎儿经治疗后存活率低。结论产前超声对双胎输血的及时诊断、治疗和提高胎儿生存率具有明显的临床价值。

Prenatal ultrasonographic features of twin-to-twin transfusion syndrome: a retrospective analysis of 30 cases

Objective To identify the clinical value of prenatal ultrasonography to predict and diagnose twin-to-twin transfusion syndrome.Methods The ultrasonographic data of 30 pregnant women of twin-to-twin transfusion syndrome including 60 fetuses who were diagnosed and followed up in our hospital from Aug.2006 to Mar.2011 were retrospectively analyzed.Results Among the 30 women with 60 fetuses,the S/D value of 17 pregnant women’s fetuses was increased,particularly in the recipient fetus.The 14 cases of recipient fetuses had enlarged heart,but the heart of one donor fetus showed same symptom.There were 8 donors attached to chorionic membrane.Among the 20 cases underwent fetoscopic treatment,15 cases had significantly changes.The fetus having fan-shaped and edge umbilical cord inlet had lower survival rate.Conclusion Prenatal ultrasonic examination is helpful to the immediate diagnosis and treatment of twin-to-twin transfusion syndrome,and has a positive effect to improve fetuses’ survival.
