腰神经根的薄层断面和CT解剖学研究

摘    要:目的阐明腰神经根在薄层断面和CT断面上椎间行程及周围结构的毗邻关系,为经皮激光椎间盘减压术的穿刺路径提供解剖学依据.方法对中国首例可视化人体腰椎间隙数据集进行总结和分析,观察腰神经椎根间行程的形态及其周围毗邻结构.选择53例经CT扫描无椎体和椎间盘病变的断面图像与薄层断面对照分析.结果首例中国可视化人体腰椎间隙,共计48层,每层厚度为1.0 mm,每层断面所显示的腰神经根椎间行程及其周围结构清晰;CT图像清晰显示腰神经根椎间行程大小、形态及周围结构.结论首例中国可视化人体腰神经根椎间行程部分为连续、完整的薄层断面标本,无节段性数据缺损,厚度均匀.腰神经根椎间走行和形态变化较大,穿刺路径与毗邻的腰神经根、上关节突、髂骨翼和血管等结构关系密切.

Abstract:Objective To study the thin cross sectional slice and CT images of the lumbar nerve root and its adjacent tissues so as to provide anatomic proof for puncture approach in percutaneous laser disk decompression. Methods Based on the analysis of the dataset of the lumbar nerve of Chinese Digitized Visible Human, the morphological characteristics of the lumbar nerve root and its adjacent structures were observed. Cross sectional images by CT scan from 53 healthy individuals without disorders in the lumbar vertebral body and intervertebral disc were compared with the thin cross sectional slices. Results The lumbar nerve of the Chinese Digitized Visible Human added up to 48 slices (1 mm for each slice). The structures of lumbar nerve and the adjacent tissues were clearly displayed on each slice. CT scan also displayed the structures of the lumbar nerve root and the adjacent tissues. Conclusion The first Chinese Digitized Visible Human has provided continuous thin cross sectional slices without segmental data loss. Spiral CT scan can display lumbar nerve root and its surrounding structures. The morphological changes of the lumbar nerve vary on different sections, so the puncture approach is closely related to the structures of the lumbar nerve root, superior articular process, wing ilium, and vascular blood.
