氢氧化钙消毒根管的临床研究

摘    要:目的 比较氢氧化钙与传统的根管消毒剂甲醛甲酚 (FC)在单根管感染牙根管内封药的消毒效果。方法 选择 84颗患慢性根尖周炎的单根管牙 ,经常规清理根管后 ,在根管内封入氢氧化钙糊剂 ( 4 4牙 )与FC( 4 0牙 ) ,分别在封药前、后的根管内取样接种 ,检测根管内细菌数量和检出率的变化 ,并观测封药前后临床症状和体征的变化 ,根管充填 ,1年后观察治疗效果。结果 根管内封药前、后细菌的检出率明显减少 ,其数量变化有显著性差异 ;两组封药后主要症状如疼痛、根尖部红肿均消失 ,而对根管有渗液、叩痛、瘘管的患牙 ,氢氧化钙的治疗效率明显高于甲醛甲酚组 ,两组间有显著性差异 (P <0 .0 1) ;完成根管治疗后一年 ,氢氧化钙的总有效率为 93 .2 0 % ,FC的有效率为 87.5 % ,两组间无显著性差异 (P >0 .0 5 )。结论 氢氧化钙是一种理想的根管消毒剂

Abstract:Objective To compare the disinfectant effects of calcium hydroxide and traditional formaldehyde and cresol(FC) on the intracanal medicament in infected single rooted teeth. Methods A total of 84 single rooted teeth with chronic apical periodontits were randomly selected for the disinfection with calcium hydroxide or FC in root canal after the root canals were cleared routinely. Samples were taken before and after root canal disinfection a week later. The number and species of bacteria in the root canal and the change of clinic signs and symptoms were observed. All the patients were followed up for one year. Results The number and species of bacteria in the root canal decreased significantly and there was significant difference between the two groups ( P <0.01). The main clinical signs and symptoms such as pain and apical swelling disappeared, but as to the teeth with permeation of effusion in root canal, knocking pain and fistula, the therapeutic effect of calcium hydroxide was better than that in FC group. One year after root canal therapy, the efficient rate was 93.20% in calcium hydroxide group and 87.50% in FC group. There was no significant difference between the two groups( P >0.05). Conclusion Calcium hydroxide is an effective disinfectant in root canal therapy.
