慢性肾病患者肾小管上皮细胞内质网应激与细胞凋亡的关系

摘    要:目的 探讨慢性肾病(chronic kidney disease,CKD)患者蛋白尿诱导肾小管上皮细胞内质网应激与细胞凋亡的关系.方法 应用免疫组织化学技术检测CKD患者肾小管上皮细胞中内质网应激标志蛋白氧调节蛋白150(cells-oxygen-regulated protein150, ORP150)和糖调节蛋白78(glucose-regulated protein 78, GRP78)的表达;TUNEL法检测细胞凋亡.对各指标与CKD患者24 h尿蛋白量进行相关性分析.结果 CKD患者肾小管上皮细胞细胞质中ORP150、GRP78的表达较正常对照组显著增强(P<0.01),且轻、中、重度蛋白尿3组各组间亦均有显著性差异(P<0.01),并与尿蛋白水平呈正相关(r=0.695,r=0.76,P<0.01);各组小管上皮细胞凋亡系数(apoptosis index,AI)均亦有显著性差异(P<0.01),并与尿蛋白水平呈正相关性(r=0.835,P<0.01).结论 CKD患者肾小管上皮细胞有内质网应激与凋亡的发生,并且其强度与蛋白尿水平呈正相关关系.内质网应激可能是大量蛋白尿引起小管上皮细胞凋亡的重要环节.
