容积CT数字减影血管成像诊断颅内动脉瘤的临床价值分析

摘    要:目的探讨颅内动脉瘤使用容积CT数字减影血管成像(VCTDSA)诊断的临床价值。方法选择我院2015年3月至2018年3月实施VCTDSA检查的疑似颅内动脉瘤患者82例作为研究对象,并均使用手术或影像学(3D-DSA)证实,其中68例患者有动脉瘤,共检出动脉瘤88个。结果VCTDSA检测与手术或影像学(3D-DSA)检查结果进行对比,发现两种检测方式在不同部位、不同大小动脉瘤检出数目,差异无统计学意义(P>0.05);VCTDSA检查动脉瘤患者灵敏度94.44%、特异度87.50%、准确率93.18%,且与手术或影像学(3D-DSA)检查结果一致性较好(Kappa>0.7);VCTDSA检查动脉瘤数目灵敏度96.70%、特异度93.33%、准确率96.23%,且与手术或影像学(3D-DSA)检查结果一致性较好(Kappa>0.7)。结论VCTDSA与手术或影像学(3DDSA)检查结果一致性较好,可用于颅内动脉瘤临床诊断,对颅内动脉瘤治疗提供一定参考价值。

Clinical Value of Volumetric CT Digital Subtraction Angiography in the Diagnosis of Intracranial Aneurysms

Abstract:Objective To explore the clinical value of volumetric CT digital subtraction angiography(VCTDSA) in the diagnosis of intracranial aneurysms. Methods 82 cases of patients suspected ad intracranial aneurysms who underwent VCTDSA examination from March 2015 to March 2018 in our study were selected for the study. All patients were confirmed by surgery or imaging(3D-DSA). Among them there were 68 cases with arterial aneurysm. A total of 88 aneurysms were detected. Results The comparison of examination results of VCTDSA and surgery or imaging(3 D-DSA) showed there were no significant differences in the quantities of aneurysms at different sites and different sizes by the two examinations methods(P>0.05). The sensitivity, specificity and accuracy rate of VCTDSA in examining patients with aneurysms was 94.44%, 87.50% and 93.18%, and the results were consistent with the results of surgery or imaging(3D-DSA)(Kappa>0.7). The sensitivity, specificity and accuracy rate of VCTDSA in detecting aneurysm quantity were 96.70%, 93.33% and 96.23%, and the results were consistent with the results of surgery or imaging(3D-DSA)(Kappa>0.7). Conclusion VCTDSA is consistent with the results of surgery or imaging(3 D-DSA). It can be used for the clinical diagnosis of intracranial aneurysms and provides some reference value for the treatment of intracranial aneurysms.
