继发性门脉海绵样变性螺旋CT的诊断

摘    要:目的分析继发性门静脉海绵样变性的螺旋CT表现特征及临床意义。方法搜集34例经临床证实的继发性门静脉海绵样变性患者的MSCT资料,所有病例均行上腹部螺旋CT平扫及三期增强扫描。10例采用多平面重组(MPR)、曲面重组(CPR),最大密度投影(MIP)等后处理方法进行血管成像。结果门静脉海绵样变性的螺旋CT主要表现为:门静脉主干或其分支扩张增粗,门静脉走行区可见迂曲扩张、紊乱的静脉血管影;肝实质动脉期一过性异常灌注;显示原发病灶。结论螺旋CT可优化门脉侧支循环显示,是诊断继发性门静脉海绵样变性的有效检查手段。

Diagnostic value of spiral CT in the diagnosis of secondary cavernous transformation of portal vein

Abstract:Objective To analyze the spiral CT features anddiagnostic value of secondary cavernous transformation of potlal vein. Methods 34 cases of secondary cavernous transformation of portal vein confirmed by clinical corroborative data were reviewed; plain scan and contrast-enhanced spiral CT were performed in all patients; 10 of 34 cases' imaging data were postprocessed with multi-planarreconstruction(MPR), contour re-establish ( CPR ) and maximum intensity projection ( MIP ) on blood vessel. Results Spiral CT features of cavernous transformation of portal vein were mainly showed: main portal vein or branch expanding, circuiting, mussy; one to pass abnor dabble in liver parenchyma at arterial phase; primary lesion. Conclusion Spiral CT is very useful to show the bypass circuit of portal vein, which is one of the most effective optima to diagnose secondary cavernous transformation of portal vein.
