磁共振弥散加权成像对诊断脊柱良恶性病变的临床应用价值

摘    要:目的探讨磁共振弥散加权成像(diffusion weight imaging,DWI)在诊断脊柱良恶性病变中的应用价值。方法选择42例脊柱良恶性病变患者进行脊柱常规MRI及弥散加权成像,分别测量各组表观弥散系数(a p parentdiffusion coefficient,ADC)、部分测量各项异性(fractional anisotropy,F A)值,并显示其弥散加权纤维束(diffusion weight tracking,DTT)。结果良性病变椎体压缩性骨折与相邻正常骨质比呈稍低信号,而恶性病变的椎体骨折呈略高信号。测得正常椎体ADC值为(0.23±0.05)×10-3mm2/s,恶性病变椎体骨折为(0.82±0.20)×10-3mm2/s,急性良性病变椎体骨折为(1.94±0.20)×10-3mm2/s。结论磁共振弥散加权成像较常规MRI成像能更好显示脊柱良恶性病变,ADC值是检测早期脊柱良恶性病变微观结构的敏感指标。
