16层螺旋CT血管造影的临床应用

目的 探讨16层螺旋CT血管造影在动脉血管和静脉血管方面的应用价值。方法 回顾性分析我院2003年12月-2004年12月用16层螺旋CT血管造影检查101例患者的影像资料。动脉血管造影由上肢静脉注入对比剂，静脉血管造影由下肢静脉注入对比剂，行三维重建。结果 101例中男68例，女33例，平均年龄45.6岁，头部CTA35例，发现动静脉畸形5例，动脉脉瘘1例，烟雾病1例，动脉瘤2例。颈部血管CTA5例，发现颈部血管迂曲者1例，椎动脉变异者1例，鼻咽瘤侵犯血管1例。胸部CTA12例，发现肺动脉栓塞1例，主动脉变异1例，永存动脉干1例。腹部CTA29例，发现肠繁膜上动脉栓塞1例，骨动脉栓塞1例。下腔静脉造影5例，发现下腔静脉瘤1例，腹主动脉瘤介入治疗后少量渗漏3例，腹腔巨大畸胎瘤推压肠纺膜血管1例。下肢静脉造CTA15例，均见下肢静脉血栓患侧显影浅谈，可见迂曲扩张的侧支循环血管影，健侧显影良好。结论 多层螺旋CT血管造影对全身各部位血管病变诊断有重要的临床应用价值。

Clinical application of 16-slices spiral CT angiography

Objective To evaluate clinical application of 16-slices spiral CT angiography(CTA) in artery and vein vessels. Methods 101 cases were retrospectively analyzedfor imaging data that were collected with 16-slices spiral CT angiography during Dec.2003-Dec. 2004. Contrast medium for artery was injected from vein of upper limb andfor vein from lower lime. A 3D reconstruction was performed. Results Of the 101cases, 68 were men and 33 were women with an average age 45.6y. In 35 cases receivingCTA of head vessel, 5 was with arteriovenous deformity, 1 with arteriovenous fistula, 1with moya moya disease, and 2 with aneurysm. In 5 cases receiving CTA of neck vessel,1 was with zigzag vessels, 1 with abnormalities in vertebral artery and 1 with vesselinvolvement due to nasopharyngeal darcinoma. In 12 cases receiving CTA of chest, 1was with embolism in superior mesenteric artery, 1 with embolism in renal artery. In 5cases receiving CTA of inferior caval vein, 1 was with aneurysm of inferior vena cava, 3with little leakage after invasive treatment of abdominal aortic aneurysm, and1 withmesenteric vessel pressed by gigantic teratoma. In 15 cases receiving CTA of lower limbvein, all were with shallow development in side suffering thrombus, accompanied bydeveloped zigzag and expanded collateral circulation. The healthy side was with normaldevelopment. Conclusion Multi-slices spiral CT angiography can play an importantpart in diagnosis of vessel lesions in all parts of body.
