眼部动脉与颈内动脉彩色多普勒超声检查在糖尿病视网膜病变早期诊断中的价值

摘    要:目的通过检测颈内动脉(ICA)与眼动脉(OA)、视网膜中央动脉(CRA)血流动力学的变化,分析2型糖尿病(DM)对大血管和微血管影响的差异,进而探讨其在糖尿病视网膜病变的早期诊断中的价值。方法应用彩色多普勒超声诊断仪,检测83例(166眼)2型糖尿病患者及20例(40眼)正常对照组的颈动脉、眼动脉和视网膜中央动脉血流动力学参数。结果2型DM患者OA、CRA血流动力学早期即有明显改变,呈低流速、高阻力型,颈内动脉的血流动力学改变较晚。结论2型DM患者彩色多普勒超声检测OA、CRA的血流动力学比检测ICA能更早发现糖尿病视网膜病变,且方法简便,结果准确可靠,值得在临床推广应用。

Abstract:Objective To evaluate the effect of type 2 diabetes mellitus (2-DM) on large and small vessels by detecting the hemodynamic changes in internal carotid artery (ICA), ophthalmic artery (OA) and central retinal artery (CRA), so as to assess the diabetic retinopathy early in DM patients. Methods 83 patients (166 eyes ) and 20 healthy control subjects were examined by color Doppler imaging. Results The hemodynamic changes appeared earlier in OA and CRA than in CA. The blood flow was of low velocity and high resistant. Conclusion Diabetic retinopathy can be detected earlier by using color Doppler imaging to examine the hemodynamic changes in OA and CRA than by that in ICA. The method is easier and reliable, and worthy of wide application in clinic.
