血管化腓骨瓣游离移植整复下颌骨缺损

目的：探讨血管化腓骨瓣游离移植整复下颌骨缺损的手术方法和疗效、方法：对2例下颌骨大部缺损患者应用血管化腓骨瓣游离移植整复．以腓动静脉为血管蒂切取腓骨瓣，根据下颌骨缺损的特点对腓骨进行截骨塑形，按照定位钢板定位后的下颌骨缺损形态和长度准确就位，用小钛板进行移植骨坚固内固定．结果：术后伤口均一期愈合，分别随访半年及2a，2例患者的外形及功能恢复均较为满意．结论：腓骨瓣是舌建下重建下颌骨缺损的理想组织瓣，其具有骨量充足，外形好，并发症少，成功率高等特点．

Application of Vascularized Fibular Free Flap to the Reconstruction of Mandibular Defects

Objective: To discuss the surgical method and curative effect of vascularized fibular free flap on the reconstruction of mandibular defects. Methods: Vascularized fibular free flaps were used to reconstruct mandibular defects in 2 cases. The fibular free flaps were harvested on the fibular vessels. They were cut into segments and shaped according to the features of the mandibular defects. Template and prepared-fixed reconstruction plate were made, the grafted bone placed precisely and then rigidly fixed with titanium miniplate. Results: The wound healed well. The 2 cases both survived well with satisfactory function and cosmetic effect. Conclusion: The application of vascularized fibular free flap to the reconstruction of mandibular defects is an ideal method because it can provide abundant bone tissue with satisfactory profile and low complication.
