依诺肝素钠治疗慢性阻塞性肺疾病血栓前状态的临床观察

目的 观察依诺肝素钠治疗慢性阻塞性肺疾病（COPD）血栓前状态（prothrombotic state,PTS）的疗效.方法 分析54例COPD伴血栓前状态患者的临床治疗,随机分为低分子肝素组和常规组两组,每组27例.低分子肝素组在常规治疗基础上加用依诺肝素钠4000 IU,1次/d皮下注射;两组疗程均为10d.比较两组治疗前后纤维蛋白原、D-二聚体水平、肺功能、指脉氧变化及症状缓解时间.结果 依诺肝素钠组治疗后症状缓解时间、指脉氧及纤维蛋白原、D-二聚体水平变化改善优于常规组（P＜0.05）.治疗后肺功能两组未见差别（P＞0.05）.并且无出血、药物过敏等副作用.结论 依诺肝素钠联用治疗COPD血栓前状态,能够迅速缓解症状、缩短住院时间、预防血栓,有效、方便、安全,没有增加风险,可以常规使用.

Clinical observation on prethrombotic state for effect of enoxaparin sodium on chronic obstructive pulmonary disease

Objective To observe the effect of enoxaparin sodium in the treatment of the prethrombotic state （ PTS ） of chronic obstructive pulmonary disease（COPD）. Methods The prethrombotie state of COPD of 54 cases in patients, were randomly divided into low molecular weight heparin group and conventional therapy group, each group of 27 cases. Low molecular weight heparin group, on the basis of conventional therapy plus 4000IU with enoxaparin sodium, 1 time per day, subcutaneous injection. Both groups were treated for 10d. Comparison of the 2 groups, before and after treatment of fibrinogen, D- dimer levels, pulmonary function, finger oxygen saturation and fibrinogen changes, symptom remission time. Results Enoxaparin sodium group after treatment, in symptom remission time, finger oxygen saturation and fibrinogen, D- dimer level is better than the conventional therapy group （ P 〈 0. 05 ）. Pulmonary function after treatment , There was no significant difference between two groups （P 〈 0. 05 ）. And no bleeding, drug allergies and other side effects. Conclusions . Enoxaparin sodium in treatment of prothrombotic state of COPD, can rapidly relieve symptoms, shorten the hospitalization time, prevent thrombosis, which is effective, convenient, safe, and no increase in risk, can be routinely used.
