妊娠期需氧菌性阴道炎对妊娠结局的影响

目的探讨需氧菌性阴道炎（AV）孕妇阴道菌群分布及与妊娠结局的相关性。方法选取2010年7月至2012年7月于深圳市南山医院产科门诊常规产检建册、诊断为AV的孕妇150例为研究组,选取同期于门诊常规产检的正常孕妇100例为对照组。取阴道分泌物涂片及微生物培养,AV阳性依患者意愿治疗。分析AV孕妇阴道菌群分布情况,随访妊娠结局。结果 AV孕妇分泌物一般为黄色或黄绿色、稀薄脓性。阴道菌群主要以GBS（B族链球菌,group B streptococcus）、大肠埃希氏菌、金黄色葡萄球菌及粪肠球菌为主。AV孕妇胎膜早破、绒毛膜羊膜炎、产后出血发生率均高于对照组（P〈0.05）。应用克林霉素治疗后,以上妊娠结局发生率明显降低。结论孕期AV可导致胎膜早破、绒毛膜羊膜炎、产后出血等,规范治疗可改善妊娠结局。值得临床推广应用。

Relationship between aerobic vaginitis in pregnant women and pregnant outcomes

Objective To investigate the aerobic vaginitis（ AV） pregnant vaginal flora distribution and correlation with pregnancy outcome. Methods July 2010- July 2012 at Nanshan Hospital of Shenzhen obstetric clinic routine check-ups to build volumes,diagnosed as AV pregnant women 150 cases for the study group,select the corresponding period in the clinic for routine prenatal care for normal pregnant women 100 cases of the control group. Took vaginal smears and microbial culture,AV-positive patients in accordance with the wishes of the treatment. Analyzed the distribution of AV maternal vaginal flora,follow-up pregnancy outcome. Results AV pregnant secretions usually resulted yellow or yellow-green, thin purulent. Vaginal flora mainly GBS（ B streptococci,group B streptococcus）,Escherichia coli,Staphylococcus aureus and enterococcus faecalis. AV pregnant premature rupture of membranes,chorioamnionitis,higher incidence of postpartum hemorrhage（ P ＆lt;0. 05）. After application of clindamycin treatment,more significantly reduced the incidence of pregnancy outcome. Conclusions AV pregnancy can lead to premature rupture of membranes, chorioamnionitis,postpartum hemorrhage; standard therapy can improve pregnancy outcomes, which is worthy of clinical application.
