结肠镜在消化系统疾病中的应用价值——附231例肠镜检查病例分析

目的探讨结肠镜在消化系统疾病住院患者中的应用价值。方法统计贵阳医学院附属医院消化内科住院患者中行结肠镜检查的结果,分析结肠镜检出各种病变所占的比例及结肠镜对其诊断、治疗作用。结果 961例消化内科住院患者中231例行结肠镜检查,检出大肠息肉119例(检出率为51.52%),106例行内镜下大肠息肉切除术(89.08%)。大肠息肉最多的是乙状结肠息肉82枚(32.54%),其次是横结肠息肉65枚(25.79%)和直肠息肉47枚(18.65%)。112例非肠道息肉患者中结肠镜总确诊率为38.39%。分别以便血26例(53.85%)、腹泻20例(50.00%)、腹痛50例(34.00%)确诊率较高。内镜下治疗主要为内镜下息肉切除,手术切除率为89.08%。结论消化系统疾病中有约1/3的患者接受结肠镜检查,其中约50%是肠息肉。结肠镜检查和治疗是诊断大肠息肉最主要的诊疗手段。在所有消化道症状中,腹痛、腹泻、便血对结肠镜检查的指导性最强。

Application of colonoscopy for patients with digestive diseases with 231 cases report

Objective To analyze the application of colonoscopy in hospitalized patients with gastrointestinal diseases.Methods Collect the results of colonoscopy in hospitalized patients with gastrointestinal diseases of guiyang medical university.Analysis the proportion of various diseases and the effect of colonoscopy in diagnosis and treatment.Results There are 231 cases underwent colonoscopy in 961 hospitalized patients,examination rate 24.03%,positive rate 96.97%.119 cases of colonic polyps(51.52%) were found,and 106 cases underwent polypectomy during endoscopy(89.08%).After sigmoid colon(32.54%) and transverse colon(25.79%),the rectum(18.65%) was the most common site for polyps.The detection rate of polyps is 38.39% in 112 patients.Moreover,the colonic positive detections for hematochezia(53.85%),diarrhea(50.00%),abdominal pain(34.00%) was higher than other symptoms.The most therapeutic Endoscopy procedure was polypsectomy endoscopically,with rate of polypectomy was 89.08%.Conclusions About one third of hospitalized patients with digestive system disease underwent colonoscopy.50.00% of them had colonic polyps.The best management of colonic polyps was endoscopic detection and therapy.In all digestive symptoms,abdominal pain,diarrhea,hematochezia was the strongest symptom indications for colonoscopy.
