肝功能异常患者血清中自身抗体检测及意义

目的 探讨肝病相关自身抗体在肝功能异常患者血清中的阳性率及其诊断价值.方法 以生物薄片马赛克技术制备的Hep-2细胞、猴肝、心肌、鼠肝、鼠肾、胃共6种组织细胞为抗原基质,间接免疫荧光法检测肝病相关自身抗体,根据荧光反应强弱判定结果.结果 80例肝功异常患者(肝脏功能异常,ALT＞50 U/L)自身抗体阳性率为11.25%,其中抗核抗体(ANA)、抗平滑肌抗体(SMA)及抗线粒体抗体(AMA)的阳性率分别为7.5%、5%和6.25%;女性患者自身抗体的检出率明显高于男性患者(P＜0.05);病毒性肝炎标志物阴性组自身抗体检出率明显高于阳性组(P＜0.05).结论 高滴度自身抗体对自身免疫性肝病的诊断有一定的价值,肝功能异常的非病毒性肝炎病人适合进行肝病相关自身抗体的检测.

Autoantibodies in Patients with Abnormal Liver Function

Objective To investigate the positive rate of autoantibodies in patients with abnormal liver function and explore the clinical significance of autoantibodies related to liver disease. Methods By using indirect immunofluorescence, 80 patients with abnormal liver function were detected for ANA, SMA and AMA. Results The positive rate of autoantibodies was 11.25% in 80 patients with abnormal liver function, and positive rate of ANA, AMA and SMA were 7.5%, 5% and 6.25% respectively, the positive rate of autoantibodies of female group was higher than that of male group(P<0.05). The positive rate of autoantibodies of non-viral hepatisis group was also higher than that of viral hepatitis group(P<0.05). Conclusion The detection of autoantibodies would be useful for the diagnosis of autoimmune liver diseases, those non-viral hepatisis patients with abnormal liver function would be suitable for detection of autoantibodies.
