原发性肾病综合征肾血流动力学多普勒超声研究

目的：探讨原发性肾病综合征（PNS）患者肾血流动力学多普勒超声变化的特征及临床意义。方法：76例经临床、肾穿病理证实的PNS患者分成两组：甲组40例（未治疗者），乙组36例（经治疗症状改善但仍未完全缓解者）。另有40例健康人作为对照组。应用脉冲多普勒超声（PW）观察肾段动脉血流的收缩期最大流速（Vs）、舒张期末速度（Vd）、平均速度（Vm）、搏动指数（PI）、阻力指数（RI）。结果：甲、乙组与对照组间三项参数（Vs、PI、RI）差异有显著性，但甲、乙组间比较差异无显著性。结论：多普勒超声可评价PNS患者的肾血流动力学变化特征，为临床PNS诊断提供一种无创性辅助检测手段。

Detection of Renal Hemodynamic Changes in Patients with Primary Nephrotic Syndrome by Doppler Ultrasonography

Objective\ To investigate the characters of renal hemodynamic changes of primary nephrotic syndrome(PNS) by Doppler ultrasonography.\ Methods\ 76 patients with PNS confirmed clinically and by renal biopsy pathology and 40 normal subjects were recruited in this study.\ Seventy\|six patients were divided into two groups: group A consisted of 40 patients without therapeutics,and group B had 36 patients who were treated with incomplete remission of symptoms.\ Five renal artery homodynamic parameters including peak systolic flow velocity(Vs), end\|diastolic flow velocity(Vd), average flow velocity(Vm), pulse index(Pl) and resistance index(Rl) were measured by pulsed\|wave Doppler(PW).\ Results\ There were significant difference in three parameters(Vs, Pl and Rl) between patients and normal subjects, without any difference in the three parameters between group A and B.\ Conclusion\ Doppler ultrasonography is useful to detect renal hemodynamic changes in patients with PNS, and helpful in noninvasive diagnosis and follow\|up of PNS.
