慢性乙型肝炎患者外周血Treg/Th17检测及其意义

目的通过观察慢性乙型肝炎(CHB)患者外周血Treg、Th17细胞及其相关细胞因子的变化,探讨淋巴细胞亚群Treg和Th17细胞在CHB发病机制中的作用。方法采用流式细胞术检测70例CHB患者(CHB组)和20例健康体检者(对照组)外周血中Treg(CD4+CD25highCD127lowT)和Th17(CD3+CD8-IL-17+T)细胞频数;采用酶联免疫吸附试验(ELISA)检测血清中IL-10、TGF-β1、IL-17及IL-23水平。结果 CHB组外周血中Treg、Th17细胞频数及其相关因子IL-23、IL-10、TGF-β1水平均明显高于对照组(P<0.05),但IL-17升高不明显(P>0.05)。CHB组Treg/Th17比率明显低于对照组(P<0.05),且轻中度CHB组Treg/Th17比率及TGF-β1水平明显低于重度CHB组(P<0.05)。结论 CHB患者外周血中存在Treg/Th17失衡;Treg/Th17失衡可能参与了CHB的发生发展。

Detection and Significance of Treg/Th17 Imbalance in Peripheral Blood of Patients with Chronic Hepatitis B

Objective To research the role of Treg and Th17 cells in the pathogenesis of chronic hepatitis B(CHB) by observing the variation of Treg,Th17 cells and Treg/Th17 cell-associated cytokines in peripheral blood of patients with CHB.Methods 70 CHB patients(CHB group) and 20 healthy volunteers(control group) were enrolled in this study.The frequencies of Treg(CD4+CD25highCD127low) and Th17(CD3+CD8-IL-17+T) cells in peripheral blood were assessed by flow cytometric analysis.Treg/Th17 cell-associated cytokines such as IL-10,TGF-β1,IL-17 and IL-23 were measured by enzyme-linked immunosorbent assay(ELISA).Results The frequencies of Treg/Th17 cells and the levels of IL-23,IL-10 and TGF-β1 in peripheral blood of CHB group were significantly higher than those of the control group(P<0.05),while the levels of IL-17 did not change markedly.The ratios of Treg/Th17 were lower than those of control group(P<0.05).Compared with the mild to moderate CHB group,the ratios of Treg/Th17 and the levels of TGF-β1 were significantly increased in the severe CHB group(P<0.05).Conclusions Treg/Th17 dysfunction exists in patients with CHB,and it may play a potential role in the onset and development of CHB.
