结肠癌肝转移高危因素探讨及其临床意义

目的　探讨结肠癌肝转移的主要危险因素及其临床意义。　方法　比较结肠癌肝转移 40例 (转移组 )及同期无肝转移结肠癌 35 7例 (对照组 )病理因素及血清癌胚抗原 (CEA)水平。　结果　结肠癌肝转移原发部位多见于乙状结肠 (37.5 % ) ,转移组淋巴结转移率高、肠壁浸润深度深 ,转移组组织学以高分化型腺癌为主 (72 .5 % ) ,转移组血清 CEA水平增高明显 ,与对照组比较差异均有显著性 (P<0 .0 5 ) ;年龄、肿瘤浸润周径、肿瘤大体分型对肝转移影响不大 ,两组差异无显著性 (P>0 .0 5 )。　结论　结肠癌肝转移多发生于乙状结肠、伴有淋巴结转移、浸润全层、高分化型癌及 CEA水平高者

Clinical Significance of Risk Factors Analysis in Colorectal Carcinoma Associated with Liver Metastasis

Objective To explore risk factors in colorectal carcinoma associated with liver metastasis and clinical significance. Methods The clinical pathological factors and carcinoembryonic antigen(CEA) serum level were contrasted between 40 cases colorectal carcinoma associated with liver metastasis and 357 cases those no associated with liver metastasis at same periods. Results Liver metastasis of colorectal carcinoma correlated with primary tumor site, lymph|node metastasis, depth of infiltration to the intestine wall, degree of histological differentiation, CEA serum level(P<0 05). Age, gross type, the involved circumference of the intestinal wall showed no obvious effect on liver metastasis(P>0 05). Conclusion Liver metastasis of colorectal carcinoma more occured in those from sigmoid colon, lymph node metastasis,deep infiltration to intestine wall and well|differentiated tumor. Paying attention to these risk factors may help to early find liver metastasis in post|operative colorectal carcinoma and upgrade survival rate and therapeutic rate.;
