腹股沟疝无张力修补术硬膜外麻醉与局部麻醉的比较

目的：比较局部麻醉和硬膜外麻醉下行腹股沟疝无张力修补术的效果。方法：对62例腹股沟疝无张力修补患者进行研究，分为局部麻醉组（n=17例）和硬膜外麻醉组（n=45例），对两组的手术时间、术后尿潴留、术后伤口疼痛、术后下床活动时间、住院费用和术后复发进行比较研究。结果：局部麻醉组在手术时间、术后尿潴留、术后下床活动时间、住院费用等方面均少于硬膜外麻醉组（P〈0．05）。结论：局部麻醉下行腹股沟疝无张力修补术术后并发症少，可早期下床活动，住院费用低，对全身器官功能影响小，尤其适合老年患者，值得推广。

The comparison of local anesthesia with epidural block anesthesia in tension-free inguinal hernia repair

Objective:To study the efficiency and benefit of tension-free inguinal hernia repair with local anesthesia compared with epidural block anesthesia.Methods:All of the 62 cases were performed tension-free hernioplasty with mesh-plug made of PP.17 cases were treated with local anesthesia and the other 45 cases were treated with epidural block anesthesia.And the operation time,postoperative complications such as retention of urine,wound pain,postoperative time staying in bed,hospitalization costs and recurrence were observed.Results: Compared with the epidural block anesthesia groups,the local anesthesia groups had shorter operation time,less postoperative complications such as retention of urine,lower costs and shorter postoperative time staying in bed(P<0.05).Conclusions: The tension-free inguinal hernia repair performed with local anesthesia compared with epidural block anesthesia had many benefits such as less postoperative complications,shorter postoperative time staying in bed and lower hospitalization costs.And it had lower side effect on the whole body,thus it is especially suitable to the old patients and is worthy to popularize.
