奥氮平联合氟西汀治疗伴自杀意念抑郁症患者的效果

目的:观察奥氮平联合氟西汀治疗伴自杀意念抑郁症患者的效果。方法:选取80例伴自杀意念抑郁症住院患者为研究对象,采用随机数字表法分为研究组和对照组各40例。对照组采用氟西汀治疗,研究组在对照组基础上联合奥氮平治疗。比较两组的抑郁量表(HAMD-17)评分、自杀意念(BSSI量表)评分、认识功能(MoCA量表)评分、治疗效果及不良反应发生情况。结果:治疗2周、4周、8周后,两组HAMD-17评分和BSSI评分均低于治疗前,且研究组均低于对照组,差异有统计学意义(P<0.05);治疗8周后,两组MoCA总分均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);研究组治疗总有效率为90.00%,明显高于对照组的67.50%,差异有统计学意义(P<0.05);研究组不良反应发生率为55.00%,与对照组的42.50%比较,差异无统计学意义(P>0.05)。结论:奥氮平联合氟西汀治疗伴自杀意念抑郁症患者,可降低抑郁评分和自杀意念评分,提高认识功能和治疗总有效率,效果优于单纯氟西汀治疗效果。

Effects of Olanzapine combined with Fluoxetine in treatment of patients with depression with suicidal ideation

Objective:To explore effects of Olanzapine combined with Fluoxetine in treatment of patients with depression with suicidal ideation.Methods:80 inpatients with depression with suicidal ideation were selected as the research objects,and were divided into control group(n=40)and study group(n=40)according to the random number table method.The control group was treated with Fluoxetine,while the study group was combined with olanzapine on the basis of that of the control group.The scores of Hamilton depression scale(HAMD-17),beckscale for suicide ideation(BSSI scale)scale and Montreal cognitive assessment scale(MoCA scale),treatment effects and adverse reactions were compared between the two groups.Results:2 weeks,4 weeks,and 8 weeks after the treatment,the HAMD-17 scores and BSSI scores of the two groups were lower than those before the treatment;those of the study group were significantly lower than those of the control group;and the differences were statistically significant(P<0.05).8 weeks after the treatment,the total scores of MoCA of the two groups were higher than before the treatment;that of the observation group was significantly higher than that of the control group;and the differences were statistically significant(P<0.05).The total clinical effective rate in the study group was 90.00%,which was significantly higher than 67.50%in the control group,and the difference was statistically significant(P<0.05).The incidences of adverse reactions in the study group and the control group were 55.00%and 42.50%,respectively,and the difference was not statistically significant(P>0.05).Conclusions:Olanzapine combined with Fluoxetine in the treatment of the patients with depression with suicidal ideation can reduce the depression score and suicidal ideation score,and improve the cognitive function and the total effective rate of treatment.Moreover,it is superior to single Fluoxetine.
