临床大肠埃希氏菌耐药性检测及超广谱β-内酰胺酶流行病学调查

摘    要:目的了解广州地区临床大肠埃希氏菌耐药及超广谱β-内酰胺酶(ESBLs)的流行情况。方法对临床分离鉴定的57株大肠埃希氏菌进行5大类15种抗菌药物的耐药性检测。通过表型确认试验筛选出产ESBLs菌株,采用PCR法检测15种临床重要且常见的耐药基因。结果 57株大肠埃希氏菌对15种抗菌药物产生了不同程度的耐药性。其中有23株产ESBLs,ESBLs产生率为40.4%;24株产TEM,13株产CTX-M(其中,8株产CTX-M-55、4株产CTX-M-14、1株产CTX-M-27),2株产OXA-1。结论广州地区大肠埃希氏菌耐药情况依然严峻。同时,ESBLs流行趋势发生了变化:以前以流行CTX-M-9型为主,现在以CTX-M-1型和CTX-M-9型同时流行为主要特征。

Abstract:Objective To investigate the antibiotic resistance profiles of clinical Escherichia coli isolates from Guangzhou and the epidemiology of ESBLs. Methods Fifty-seven Escherichia coli isolates were underwent susceptibility test of 15antimicrobial agents in 5 major categories and screening tests for ESBLs. ESBLs positive isolates were analyzed for the 15drug-resistance associated genes by PCR. Results Fifty-seven clinical Escherichia coli isolates exhibited different levels of resistance to the tested antimicrobial agents.Twenty-three were confirmed of producing ESBLs,including 24 TEM,13 CTX-M,and 2 OXA-1.Among the 13 CTX-M producers,CTX-M-1 group(8 producing CTX-M-55)and CTX-M-9 group(4 producing CTX-M-14)were the predominant genotypes. Conclusion The drug resistance of clinically isolated Escherichia coli strains in Guangzhou remains challenging and there is a significant change in the prevalence of ESBLs :CTX-M-1 group and CTX-M-9group have become dominant ESBLs while CTX-M-9 group was the prevailing resistant type in the past.
