体外膜肺氧合并发症研究

目的体外膜肺氧合（ECMO）并发症可分为机械系统并发症和患者机体并发症，机械系统并发症包括氧合器衰竭、循环管道破裂，以及驱动泵和热交换器功能异常，患者机体并发症包括出血、神经系统并发症、器官功能衰竭和感染等。研究的目的是了解ECMO支持患者并发症发生情况。方法分析总结了阜外医院从2004年到2005年所有ECMO支持患者的临床病例资料，研究了18例ECMO支持患者的并发症发生率。结果18例患者存活10例，生存率为55．6％；死亡8例，死亡率为44．4％。ECMO支持患者出现了出血、感染、肾功能衰竭、神经系统并发症，和氧合器衰竭，发生率依次为44．4％，27．8％，16．7％，11．1％和27．8％。结论ECMO支持时间越长，其发生并发症的危险性就越大，出血仍是ECMO最主要的并发症。

Study of Complications On Extracorporeal Membrane Oxygenation

OBJECTIVE The complications associated with extracorporeal membrane oxygenation(ECMO) can be classified into mechanical and patient-medical complications.Mechanical complications include oxygenator failure,tubing disruption,pump and heat exchanger malfunction.Patient-related medical problems are bleeding,neurological complications,organ failure and infection.The objective of this study was to investigate the complications of ECMO.METHODS The authors reviewed clinical records of the ECMO patients at Fuwai Hospital from 2004 to 2005.A total of 18 patients with the ECMO supported in our hospital were studied for the incidence of complications.RESULTS Among 18 patients with ECMO,10 patients were cured with the rate of 55.6%,and 8 patients were died with the mortality as 44.4%.They suffered the complications,such as bleeding,infection,renal failure,neurological complications and oxygenator failure with the incidence of 44.4%,27.8%,16.7%,11.1 % and 27.8%respectively.CONCLUSION Generally,the risk of complications increases with the duration of time on ECMO support.Bleeding is still the most important complication of ECMO.
