| 摘 要： | 目的报告全国首例单孔腹腔镜手术(LESS)治疗妇科恶性肿瘤,并探讨LESS在妇科微创领域中的应用。方法 57岁女性,因绝经后阴道流血5年入院,诊刮病理确诊为子宫内膜腺癌。行LESS子宫内膜癌分期手术,取脐轮上方弧形切口约2.5 cm,置入单孔多通道套管(Tri-port)建立手术通路。结果手术成功完成,手术总时间4.5 h,LESS部分4.0 h,其中建立手术通路部分1.0 h。术中没有增加其它通道,没有中转开腹。术中出血约100 ml。术后2 d即恢复肠蠕动,拔除尿管后即恢复排尿功能。术后4 d拔除腹腔引流管,无阴道流血。脐部切口呈II/甲愈合,与脐轮完美融合一体。结论随着手术器械的改进以及手术技巧的提高,在严格把握手术适应证的前提下,更为微创化的LESS有望成为治疗妇科良恶性疾病的有效的方法之一。
