148例鼾症及阻塞性睡眠呼吸暂停低通气综合征危险因素的分析

 目的 探讨年龄、肥胖、Epworth嗜睡量表（Epworth sleepiness score，ESS）评分等指标与鼾症和阻塞性睡眠呼吸暂停低通气综合征（obstructive sleep apnea hypopnea syndrome，OSAHS）发病及严重程度的相关关系。方法 对2008年5~7月因睡眠时打鼾就诊的148名患者进行问卷调查和主观评分，进行至少1次的夜间多导睡眠图监测（polysomonography，PSG），并记录身高、体重，计算体重指数（BMI）。年龄、BMI、最低SaO2（%）、ESS评分、最大氧减（%）、总暂停时间、平均呼吸紊乱指数（respiratory disorder index，RDI）之间相关关系应用Pearson correlation检验计算；单纯鼾症组及OSAHS轻、中、重各组间比较采用广义线性模型分析。结果 OSAHS患者中比例男性高于女性，随年龄增长发生率呈递增趋势。ESS评分、BMI、最低SaO2与RDI在统计学上有非常显著的相关关系，且以上因素与OSAHS的严重程度亦有明显的相关关系（P<0.05）。结论 OSAHS在打鼾就诊人群中患病率高，年龄、肥胖是易患因素。BMI、ESS评分、最低SaO2与RDI有良好的相关性，临床上可作为初步判断OSAHS患者病情的指标。
