慢性阻塞性肺病患者口腔闭合压与同步胸腔压的比较

摘    要:通过对18例慢性阻塞性肺病(COPD)缓解期患者(I组)和15例呼衰气管插管患者(II组)口腔和气管闭合压(Pm0.1和PT0.1)及胸腔压(Pes)的同步测定,显示两组患者吸气后,口腔或气管插管外口压力变化明显落后于胸腔压的变化,其落后时间在Ⅰ组和Ⅱ组分别为80ms及150ms。口腔或气道外口出现吸气负压100ms时胸腔压的改变(P0.1es)明显高于Pm0.1或PT0.1。I组Pm0.1与同步胸腔压的改变(iso-timePes0.1)及吸气后100ms胸腔压的改变(Pes0.1)无明显差别,但II组PT0.1比iso-timePes0.1及Pes0.1均低(P<0.05)。

Abstract:Month,tracheal occlusion pressure (pm0.1 or PT0.1) and esophageal pressure (Pe)were recorded simultaneously in 18 stable COPD patients(group I) and 15 intnbated COPD petients (gronp II). The results showed that the mouth and traeheal negative pressure lagged behind the esophageal pressure swing 80 ms and 150ms in group I and group II respectively after inspiring against a closed airway. In fact,Pm0.1 and PT0.1 just reflects the change of pressure in the outer end of the airway at 180 ms and 250 ms respectively after the onset of Pe changes,and were much lower than Pe change during this perlod. Pm0.1 was neariy the same an the iso-time Pes(iso-time Pes0.1) and esophageal pressure at 100ms after the onset of inspiration (Pes0.1) in group I, but PT0.1 in group II was greater than the iso-time Pes0.1 and Pes0.1
