| 摘 要： | 目的 研究BCL-6基因重排在广西结外弥漫性大B细胞淋巴瘤(DLBCL)不同免疫类型中的发生情况及与预后的关系.方法 应用免疫组化法对126例结外DLBCL进行CD10、BCL-6、MUM1、Ki67检测,采用Hans免疫分型方法将结外DLBCL分为生发中心样(GCB样)和非生发中心样(非GCB样)两个免疫亚型;应用荧光原位杂交技术(FISH)检测BCL-6基因重排.结果 126例DLBCL中,GCB样型48例(38.1%),非GCB样型78例(61.9%).BCL-6基因重排阳性率42.9%(54/126),其中GCB样型的BCL-6基因重排阳性率31.3%(15/48),非GCB样型的BCL-6基因重排阳性率50.0%(39/78),非GCB样型的基因重排阳性率高于GCB样型,差异有统计学意义(P＜0.05);无论是在GCB样型还是在非GCB样型中BCL-6基因重排与Ki67表达均未表现出相关性(P＞0.05).结论 结外DLBCL以非GCB样免疫类型占多数,两种亚型中均有一定比例发生BCL-6基因重排,其中预后较差的非GCB样型发生该基因重排的概率更高,提示该基因异常对结外DLBCL患者的预后可能有一定的影响.在GCB样型和非GCB样型中,BCL-6基因重排与Ki67表达均未表现出相关性,提示BCL-6基因重排可能并未对结外DLBCL肿瘤细胞的增殖指数起到直接或显著的影响.
