摘    要:目的探讨CCN2及APC蛋白在结直肠癌组织中的表达及临床意义。方法采用免疫组化SP技术检测正常结直肠黏膜组织、癌旁组织、结直肠腺瘤、非转移性结直肠癌、转移性结直肠癌(metastatic colorectalcancer,mCRC)及远处转移癌组织各30例中CCN2蛋白及APC蛋白的表达情况;结合结直肠癌患者的临床病理参数进行分析。结果非转移性结直肠癌、mCRC及远处转移组织中CCN2蛋白的表达率分别为76.67%、73.33%和70.00%;而在癌旁组织、结直肠腺瘤和正常结直肠黏膜组织中的表达率分别为6.67%、23.33%和0.00%,差异有统计学意义(2=74.263,P=0.000);APC蛋白在非转移性结直肠癌、mCRC及远处转移组织中均表现为低表达(表达率分别为26.67%、23.33%和16.67%),在癌旁组织、结直肠腺瘤、正常结直肠黏膜组织中均表现为高表达,分别为86.67%、76.67%和90.00%,差异有统计学意义(2=70.00,P=0.000);CCN2表达与结直肠癌患者年龄、性别、肿瘤最大径和肿瘤组织病理分型无明显关系(P>0.05),而与肿瘤的TNM分期及5年生存率相关(分别为2=4.689,P=0.03;2=4.13,P=0.042);APC蛋白表达与CCN2表达呈明显负相关(rs=-0.467,P=0.001)。结论 CCN2基因是一种癌基因,CCN2在结直肠癌中表达与结直肠癌临床病理指标有关系,CCN2可考虑作为诊断结直肠癌或判断结直肠癌预后的指标;CCN2在结直肠癌中表达与APC有关,且可能受Wnt信号通路的调节。

Abstract:Objective To investigate the roles of CCN2 and APC in colorectal cancer.Methods Immunohistochemical staining of CCN2 and APC were performed in 180 samples,including each 30 samples of normal rectum mucosa tissue,adjacent noncancerous tissue,adenoma of colorectum,non-metastatic colorectal carcinoma,metastatic colorectal carcinoma and distant metastasis tissue.Results The positive rates of CCN2 in non-metastatic colorectal carcinoma,metastatic colorectal carcinoma and distant metastasis tissues were 76.67%,73.33% and 70.00%,respectively,which were significantly higher than those in the mucosa tissue of normal rectum,adjacent noncancerous tissue,and adenoma of colorectum(X2=70.00,P=0.000).The positive rates of APC in adjacent noncancerous tissue,adenoma of colorectum and mucosa tissue of normal rectum,were 86.67%,76.67% and 90.00%,respectively,which were significantly higher than those in the non-metastatic colorectal carcinoma,metastatic colorectal carcinoma and distant metastasis tissue(X2=70.00,P=0.000).The expression of CCN2 was significantly correlated with the TNM neoplasm phasing and five-year survival(X2=4.689,P=0.03;X2=4.13,P=0.042),while the expression of APC and CCN2 was significantly negatively correlated(rs=-0.467,P=0.001).Conclusion CCN2 gene is a oncogene,of which expression is correlated with the clinical pathology of colorectal cancer.The expression of CCN2 in colorectal cancer may be regulated by Wnt signal pathway.
