高龄胃十二指肠急性穿孔患者12例临床分析

目的探讨高龄胃十二指肠急性穿孔患者的临床特点及治疗措施。方法对12例高龄胃十二指肠急性穿孔患者的临床资料进行回顾性分析。结果胃溃疡穿孔3例,十二指肠球部溃疡穿孔8例,胃癌穿孔1例。所有患者都合并一种及以上基础疾病。所有患者均行手术治疗,其中单纯穿孔修补术11例,胃癌根治术1例,死亡3例。结论高龄患者胃十二指肠急性穿孔有其特殊的临床特点,治疗以单纯穿孔修补手术为主,术后并发症多,死亡率高,但通过及时诊断及手术治疗,同时加强围手术期的处理,可以显著提高患者生存率。

Clinical Analysis on Acute Gastroduodenal Perforation in 12 Advanced Age Patients

Objective To investigate the clinical feature and surgical treatment of acute gastroduodenal perforation in advanced age patients. Methods The clinical data of 12 cases were submitted to a tetrospective study. Results There were 3 cases of gastric ulcer,8 cases of duodenobulbar ulcer and 1 case of gastric cancer.All cases had one or one more coexisting diseases.All patients were received surgical treatment： 11 patients underwent simple perforation repair and the other underwent radical gastreetomy,and 3 patients were dead after operations. Conclusion Advanced age patients with acute gastroduodenal perforation have specific clinical features and simple perforation repair is the major treatment,which encounters high risk of postoperative morbidity and mortality.It is important to increase the survival rate by timely diagnose and surgery and good perioperative treatment.
