全自动换血治疗新生儿重症溶血病

目的 :研究全自动换血疗法治疗新生儿重症溶血病的方法、疗效和安全性。方法 :对确诊为新生儿重症溶血病的2 0例患儿采用末梢动、静脉留置针穿刺 ,形成血液换出及输入同步回路。静脉端采用推注泵控制输血速度 ;动脉端由推注泵控制抗凝用的肝素生理盐水输入速度 ,肝素生理盐水经三通汇合后一起随换出血液流至排血管 ,同时用输液泵控制排血速度 ,换血在密闭的系统中全自动完成。监测换血前后的血常规、红细胞压积、血胆红素、电解质、血糖。结果 :换血前后血红蛋白、红细胞压积、血电解质、血糖无显著性差异 (P>0 .0 5 ) ;但血清总胆红素、间接胆红素、直接胆红素在换血前后有非常显著性差异 (P<0 .0 1 )。结论 :全自动动、静脉同步换血治疗新生儿重症溶血病效果好 ,副作用少 ,操作简单 ,值得推广。

Automated exchange transfusion for treating hemolytic disease of the newborn

Objective:To explore the method,efficacy and safety of automated exchange transfusion for treating severe hemolytic disease of newborn.Methods: 20 cases of severe hemolytic disease of newborn were carried out the puncture in peripheral arteriovenous vascular and established simultaneous blood import and export road.In venous side,the speed of blood transfusion was controlled by a pump device.In arterial side,0.9% of sodium chloride with heparine was transfused by another pump device.All blood exchange process was finished in a closed device system automatically.Rt.blood,blood bilirubin,electrolytes and blood glucose were detected.Results:Before and after blood exchange,no significant difference of hemoglobin,hematocrit,blood electrolytes and blood glucose were found(P>0.05).However,the significant difference of total bilirubin,conjugated bilirubin and unconjugated bilirubin were found (P<0.01) before and after the blood exchange.Conclusion:Automated exchange transfusion for treating severe hemolytic disease of the newborn has the features of good effect,less side effect,simple performance and it is worth of using widely.
