“胎数落”病因病机及针灸诊疗思路探析

现代中医学认为复发性流产相当于中医学中的"滑胎""胎数落"。但我们在探究复发性流产的中医学病名起源中发现,"滑胎"一词最早的含义与当今所指的复发性流产不同,"滑胎"在清乾隆四年以前实为临产催生的方法,而"数堕胎""胎数落"才更接近复发性流产的含义。历代中医对"胎数落"在发病原因及发病机制方面的认识上有寒热之分、虚实之别,各有千秋,自成体系,尤其对自然流产频率的连续性即"应期而落"这一特点的认识相当精确。针灸在治疗"胎数落"时须做到明辨虚实,虚则益气血、补脾肾、调冲任,实则活血化瘀兼以理气通经。

Analysis on Etiology and Pathogenesis of Repeated Abortion and Diagnosis and Treatment of Acupuncture

Modern Chinese medicine considered that the recurrent abortion is equivalent to“habitual abortion”and “repeated abortion” in the traditional Chinese medicine. In the study of the origin of the names of recurrent abortion, it has found that the earliest meaning of the word“habitual abortion”was different from the current abortion.“Habitual abortion”was actually a method of birth induction before Qianlong four years of Qing Dynasty,while “repeated abortion”is closer to the meaning of recurrent abortion. The understanding of the etiology and pathogenesis of“recurrent abortion”in ancient Chinese medicine is divided into cold and heat,deficiency and excess, each with its own pros and cons, especially the understanding of the continuity of the frequency of spontaneous abortion,that is,“fall at the prospective timing”. It is important to identify whether it is deficiency or excess when applying acupuncture to the treatment of“recurrent abortion”. The treatment focuses on nourishing qi and blood, invigorating spleen and kidney and regulating chong and conception channels when it is deficiency syndrome,and activating blood and remove stasis while regulating qi and promoting menstruation when it is excess syndrome.
