本院肺炎克雷伯菌的临床分布及耐药性分析

摘    要:目的:调查分析笔者所在医院2010—2012年肺炎克雷伯菌的临床分布及耐药情况,以指导临床合理使用抗菌药物。方法:采用回顾性方法收集笔者所在医院2010—2012年临床各类标本分离的肺炎克雷伯菌165株,对其药敏试验结果进行统计、分析。结果:肺炎克雷伯菌在病原菌中的检出率为2.14%,165株肺炎克雷伯菌对常用抗菌药物均产生不同的耐药性,这3年平均耐药率〉50%的有氨苄西林(94.2%)、头孢噻吩(69.1%),头孢呋辛(64.5%)和庆大霉素(52.7%);耐药率较低的抗菌药物是环丙沙星(7.8%)、赈拉西林/他唑巴坦(8.3%)和阿卡米星(8.9%)等,对亚胺培南和美罗培南耐药率最低(O%);肺炎克雷伯菌对头孢曲松、头孢吡肟和氨曲南耐药率呈现逐步上升趋势。结论:肺炎克雷伯菌的耐药性呈上升趋势,应加强抗菌药物管理,临床治疗应依照药敏试验结果选用抗菌药物。

Clinical Distribution and Resistance Analysis(Drug Resistance)of Klebsiell Pneumoniae in Our Hospital

Abstract:Objective: To investigate and analyse the clinical distribution and the drug resistance of Klebsiella pneumoniae from 2010 to 2012 so as to guide reasonable use of antibiotics in clinical.Method: A total of 165 strains of Klebsiella pneumoniae isolates were collected from 2010 to 2012 retrospectively.Count and analyse the result of their drug sensitive tests.Result: The detection rate of Klebsiella pneumoniae is 2.14% in the pathogen. Different drug resistance to frequently-used antibiotics occurred among in the 165 strains.Drugs which the average resistance rate above 50% in the three years were ampicillin ( 94.2% ), cephalothin ( 69.1% ), cefuroxime ( 64.5% ), gentamicin ( 52.7% ) .Drugs which the resistance rates were much lower were ciprofloxacin ( 7.8% ), tazobactam sodiundpiperacillin sodium ( 8.3% ) and amikacin ( 8.9% ) .The resistance rates to impenem and meropenem were the lowest ( 0% ) ; the resistance of Klebsiella pheumoniae to Ceftfiaxone, cefepime and aztreonam were growing up gradually.Conclusion: The antimierobial resistance of Klebsiella pheumoniae has increased.The management of antibiotics should be strengthened and it' s important to choose appropriate antibiotics according to the results of drug sensitive tests.
