心包腔置ARROW导管引流治疗心脏压塞17例体会

目的探讨心脏压塞经皮心包腔内置入ARROW导管治疗的临床疗效及安全性。方法对我院明确诊断为心脏压塞的17例患者行心包腔内置ARROW管引流治疗,术后观察临床症状、引流状况及实验室检查等指标变化情况。结果所有患者置管成功,术后心脏压塞症状即刻或24 h内缓解,除1例原发心脏肿瘤置管30 d外,其它患者置管3~7 d,2例出现引流不畅或堵塞,1例化脓性心包炎术后第2天转胸外科手术治疗。结论心脏压塞置入ARROW导管引流具有创伤小、操作简便安全、疗效较好等优点,具有临床应用价值。

Percutaneous Pericardial Drainage With ARROW Catheter for Patients with Cardiac Tamponade: Clinical Analysis of 17 Cases

Objective To evaluate the clinical efficacy and safety of percutaneous pericardial drainage with ARROW catheter for patients with cardiac tamponade.Methods 17 patients with cardiac tamponade(diagnosed by clinical symptoms,lab and relative examinations) were treated by percutaneous pericardial drainage with ARROW catheter.After operation,the clinical symptoms,character of drain and results of lab examination were observed.Results Operations were successful in all patients.The symptoms of cardiac tamponade were relieved after operation immediately or in 24 hours.The catheter was placed in one patient with cardiac tumor for 30 days.In other patients,the catheters were placed for 3 to 7 days.Inadequate drainage and clog of catheter occurred in 2 patients,and 1 patient had to received surgical operative treatment,because of suppurative pericarditis.Conclusions Percutaneous pericardial drainage with ARROW catheter is a simple,effective and safe treatment for cardiac tamponade.So it is worth to be applied to treating cardiac tamponade.
