床边插管腹膜透析在慢性肾脏病发生急性肾衰中的应用

目的探讨床边插管腹膜透析（PD）治疗慢性肾脏病发生急性肾衰（A／C）的疗效及安全性方法回顾性分析我院在综合治疗基础上联合床边插管PD治疗26例A／C患者的疗效与转归，观察患者PD治疗前后血肌酐（Scr）、尿素氮（BUN）、钾（K^＋）、二氧化碳结合力（CO2CP）水平的变化以及床边插管PD的并发症、结果大多数A／C患者经PD治疗后短期内血BUN、Scr、K^＋、CO2CP下降非常明显（P〈0．01），18例患者肾功能恢复到基础水平，脱离透析；8例患者肾功能无明显改变，其中1例进入维持性透析、无1例因插管出现严重的并发症、结论床边插管PD治疗A／C是一种安全、简单、经济、有效的治疗方法，早期、持续地行PD治疗有利于改善A／C的预后。

Application of Peritoneal Dialysis by Bedside Intubation on Patients with Acute Renal Failure on Chronic Kidney Disease

Objective To evaluate the feasibility and efficacy of peritoneal dialysis(PD) by bedside intubation in the treatment of patients with acute renal failure on chronic kidney disease(A/C).Methods The clinical data of 26 cases with A/C treated with PD by bedside intubation based on the routine therapy were analyzed retrospectively.The level of blood urea nitrogen (BUN),serum creatinine (Scr),kalium (K ),and carbon dioxide combine power(CO2CP)before and after the treatment of PD were detected.The safety of PD patients was observed as well.Results The level of BUN,Scr,K ,CO2CP in a majority of patients decreased significantly in a short time after treatment of PD (P<0.01).The renal function was improved to the primary levels in 18 cases,not in 8 cases,and 1 case became maintenance dialysis.No serious complication caused by bedside intubation was found.Conclusions PD by bedside intubation is a safe,simple,economic and effective treatment for the patients with A/C.Early continues PD can improve the prognosis of the patients with A/C.
