血浆循环DNA在原发性干燥综合征和淋巴瘤中的比较分析

摘    要:目的 比较血浆循环DNA(circulating DNA,cDNA)在原发性干燥综合征(primary Sj(o)gren's syndrome,pSS)和淋巴瘤(Lymphomas)中的差异.方法 选用人类基因组管家基因GAPDH设计引物,测定pSS、淋巴瘤及正常对照组中血浆cDNA的值,同时测定血沉(ESR)、C-反应蛋白(CRP)、免疫球蛋白(IgA、IgG、IgM),并评定pSS的活动指数(Sj(o)gren's syndrome disease activity index,SSDAI)和损害指数(Sj(o)gren's syndrome disease damage index,SSDDI)、淋巴瘤国际预后指标(Lymphoma international prognostic index,IPI),分析cDNA与相关参数之间的关系.结果 3组间血浆cDNA两两比较:pSS组与淋巴瘤组、正常对照组比较差异均有统计学意义(P<0.05);pSS组中:cDNA浓度值与IgG呈正相关(r=0.333,P<0.05),cDNA浓度值与SSDAI呈正相关(r=0.551,P<0.01),cDNA浓度值与SSDDI无相关性(r=0.198,P>0.05),淋巴瘤组cDNA浓度值与IPI正相关(r=0.596,P<0.01);pSS组IgA、IgG均明显高于淋巴瘤组(P<0.05).结论 血浆cDNA在pSS及淋巴瘤中均升高,且两者之间有差异,作为pSS与淋巴瘤疾病鉴别的一种新的生物学标记,也可作为淋巴瘤预后及pSS疾病活动的观察指标,或许还可作为pSS向淋巴瘤发展的监测指标之一.

Abstract:Objective To explore the significance of peripheral blood plasma circulating DNA (cDNA) in groups of prima- ry Sj(igren' s syndrome(pSS) and Lymphomas. Methods The primer was designed by human genome housekeeper gene GAPDH. Levels of cDNA, erythrocyte sedimentation rate ( ESR), C-reaction protein (CRP) and immunoglobulin ( IgA, IgG and IgM) in each group were detected. Meanwhile, the activity index and damage index of pSS, and Lymphoma interna- tional prognostic index (IPI) were assessed, and the relations between cDNA and disease activity were analyzed. Results The level of cDNA in pSS group ( 368.24 ± 196.68 ) ng/ml was significantly higher than that in Lymphoma group (20.46 ± 1.50) ng/ml, P 〈 0.05, and the cDNA level in Lymphoma group was significantly higher than that in con- trol group (24.56 ± 16.76 )ng/ml, P 〈 0.05. In pSS group : cDNA level was positively correlation with IgG (r = 0. 333, P 〈 0.05 = and Sjogren' s syndrome disease activity index( SSDAI), r = 0.551, P 〈 0.01. But there was no correlation between cDNA concentration and Sjogren' s syndrome disease damage index ( SSDDI), r = 0. 198, P 〉 0.05. In Lymphomas group: cDNA level positively correlate with IPI(r =0. 596,P 〈0.01 ) ;the levels of IgA and IgG in pSS group were apparently higher than those in Lymphoma group (P 〈 0.05 ). Conclusion The cDNA level may be a new biological marker for dis- tinguishing pSS from Lymphoma. The cDNA level can be considered as a prognostic indicator for lymphoma, moreover, it could be an observability index for pSS disease activity and the development of pSS for Lymphoma.
