原发性胆汁性肝硬化并发真菌感染2例并文献复习

目的 分析2 例并发真菌感染的原发性胆汁性肝硬化(PBC) 患者的临床、影像学和病理特征.方法 回顾性分析我院风湿科收治的2 例PBC 并发真菌感染患者的临床、影像学和病理资料,并复习相关文献.结果 2 例均为女性,1 例诊断为PBC,1 例为干燥综合征合并PBC ;2 例均应用激素及免疫抑制剂后并发真菌感染,1 例经痰PCR 检测到肺孢子虫,另外1例行肾脏穿刺病理检测后明确真菌感染的诊断.2 例病情均进展较快,最终死亡.结论 PBC 并发真菌感染预后较差,临床长期用激素和免疫抑制剂的患者需提高警惕.

Primary biliary cirrhosis accompanying fungal infection involvement: A report of 2 cases and review of the literature

Objective To analyze the imaging,clinical and pathological features of 2 patients with primary biliary cirrhosis(PBC) accompanying fungal infection.Methods Imaging,clinical and pathological features of 2 patients with PBC accompanying fungal infection were retrospectively analyzed with the related literature reviewed.Results Of the 2 female patients,1 was diagnosed as PBC and 1 as Sjogren syndrome accompanying PBC.Fungal infection occurred in 2 patients after treatment with glucocorticosteroid and immunosuppressive agents.Peumocystis was detected in sputum of 1 patient by PCR and fungal infection was diagnosed in 1 patient by renal needle biopsy.The 2 patients died due to the rapid progress of their disease.Conclusion The prognosis of patients with PBC accompanying fungal infection is poor.Precautions should be concerned for those receiving long-term treatment with glucocorticosteroid and immunosuppressive agents.
