冠状动脉造影对平板运动试验诊断冠心病的评价

目的　评价平板运动试验 (TET)对冠心病的诊断价值。方法　选择拟诊为冠心病的 87例患者 ,同期先后行TET和冠状动脉造影 (CAG) ,以CAG的结果为标准。分析TET的敏感性和特异性 ,并对假阴性、假阳性的临床特点进行分析。结果　TET诊断冠心病的敏感性为 77.0 8% ,特异性为 66.67% ;冠心病中假阴性为 2 2 .92 % ;冠心病患者中真阳性组的单支病变也显著低于假阴性组 (P <0 .0 1) ,其年龄比假阴性组明显增高 (P <0 .0 5 ) ;非冠心病患者中的假阳性组女性比例明显高于真阴性组 (P <0 .0 5 )。结论　TET诊断冠心病的敏感性和特异性相对较低 ,如能除外更年期综合征、心肌肥厚、糖尿病及药物等因素影响 ,可提高其准确性 ;TET对冠心病危险分层较诊断更具临床意义。

The coronary angiography in evaluation of TET for diagnosis of coronary disease

Objective To evaluate the treadmill exercise test (TET) in diagnosis of coronary disease. Methods 87 patients early diagnosed as coronary heart disease were given TET and coronary angiography (CAG), used the outcome of CAG as criteria. The sensitivity and specificity of TET, and the clinical features of false positive, false negative were analyzed. Results The sensitivity of TET in diagnosis of coronary disease was 77.08%, the specificity was 66.67%; the false negative of coronary disease was 22.92%. In patients with coronary disease, the single branch lesions in the positive group was also statistically lower than that in the false negative group (P<0.01), the disease age was also higher (P<0.05). In patients without coronary disease, the female incidence rate in the false positive group was higher than that in the negative group (P<0.05). Conclusion The sensitivity and specificity of TET are relatively low in diagnosis of coronary disease, the accuracy can be increased if there were no interferences of climacteric syndrome. myocardial hypertrophy, diabetes mellitus and drug factors. TET is of clinical significance in diagnosis of coronary disease stratification of risk.
